Delia had not sat all the way through a movie in six years.
She had also never once mentioned it to a doctor.Y tampoco se lo había mencionado nunca a un médico.
That second part is the one I want you to sit with, because it is the whole reason this condition stays hidden for years at a time. What Delia had did not sound like a symptom. It sounded like a personality. She was "the fidgety one." She was "bad at relaxing." Her family had said it so many times, so affectionately, that she had stopped hearing it as anything but a fact about who she was.
It was not. It had a name, a body clock, and — in a great many people — an ingredient that can be measured with a blood test.
The seat she always choseEl asiento que siempre elegía
Delia is fifty-eight and has run the same small-town library circulation desk for twenty-two years. She came to see me for something else entirely — a blood pressure check — and the leg story came out at the very end, the way the important things usually do, with one hand already on the door.
Here is what she told me, and I am going to give it to you in her words, because her words are better than mine:Esto fue lo que me dijo, y se lo voy a dar en sus palabras, porque sus palabras son mejores que las mías:
"It doesn't hurt. I want to be clear about that, because I know 'it doesn't hurt' makes it sound like nothing. It's more like there's something inside my legs that has to get out. Like soda fizzing in the bones. And the only thing that answers it is walking. So I walk. At eleven at night, I walk around my own kitchen island like a crazy person.""No duele.
Then she told me the part that told me the most. Over the years she had quietly rearranged her whole life around it. She stood at the back at church. She took the aisle seat at every movie, and then stopped going. She had not been on an airplane since 2020 — not because of anything you would guess, but because a seat you cannot leave is the worst place on earth when your legs are asking.
Nobody had ever asked her about it. And she had never volunteered it, because for six years she had assumed that what she was describing was not a medical thing.
First, let's clear up the biggest mix-up: this is not a crampPrimero, aclaremos la confusión más grande: esto no es un calambre
I wrote a whole article about the 2 a.m. charley horse, and readers still write to me describing something that is clearly not a cramp at all. So let's separate them properly, because they are two completely different animals that happen to live in the same neighborhood — your legs, at night.
A cramp is a muscle problem. One muscle grabs hold and will not let go. It is sudden, it is sharp, you can usually feel the hard knot with your hand, and stretching releases it. It is over in seconds to a few minutes.
Restless legs syndrome is a nervous-system problem. There is no knot. There is often no pain at all. What there is instead is an urge — an almost impossible-to-describe need to move — usually with strange sensations underneath it. MedlinePlus lists the words people reach for: creeping, crawling, tingling, burning. My own patients have said "fizzy," "electric," "like ants," "like something crawling under the muscle," and — my favorite, because it is so exact — "like my legs are bored."El síndrome de piernas inquietas es un problema del sistema nervioso. (MedlinePlus)
And here is the single cleanest way to tell them apart, the one I use at the bedside:Y aquí está la forma más limpia de distinguirlos, la que uso junto a la cama del paciente:
A cramp is relieved by stretching. Restless legs is relieved by moving — and it comes right back when you stop.
The four questions that make the diagnosisLas cuatro preguntas que hacen el diagnóstico
Here is something that surprises people: there is no scan and no blood test that says "you have restless legs syndrome." It is diagnosed from the story. Which means the story has to be told — and that is exactly where it falls apart for so many people, because they never think to tell it.Aquí hay algo que sorprende a la gente: no hay ninguna imagen ni análisis de sangre que diga "usted tiene síndrome de piernas inquietas."
Cleveland Clinic lays out the same four features doctors look for. Read them as four questions about yourself:Cleveland Clinic expone las mismas cuatro características que los médicos buscan.
- Is there an urge to move your legs, usually with an uncomfortable feeling you struggle to put into words?¿Hay una necesidad de mover las piernas, normalmente con una sensación incómoda que le cuesta poner en palabras?
- Does rest bring it on? Sitting or lying down starts it or makes it worse — not activity.¿El reposo lo provoca?
- Does moving make it better, at least while you keep moving?¿Moverse lo mejora, al menos mientras se sigue moviendo?
- Is it worse in the evening or at night than during the day?¿Es peor en la tarde-noche o de madrugada que durante el día?
Four yeses is a conversation to have with your doctor — who also has to check that something else isn't causing it, which is a real and important part of the process.
And this is not rare. MedlinePlus puts it at an estimated 5 to 10 percent of adults in the United States (and 2 to 4 percent of children), more common in women than men, and more common as people get older.Y no es raro. (MedlinePlus Genetics)
Think about that in a room. Ten people at a family dinner. Statistically, that is likely one of them — standing up "to stretch" for the third time, apologizing for being restless.
Why it waits until the day slows downPor qué espera a que el día se calme
This is the part that convinces skeptical family members, so I want to give it to you carefully.
Restless legs is not random, and it is not "because you're stressed at night." It follows a daily rhythm. Symptoms are typically mildest in the morning and build through the evening into the night. That is a fingerprint of the body's internal clock — the same clock that runs your sleep, your temperature, your hormones. Conditions caused by worry do not keep such a tidy schedule.
And there is a second trap layered on top of it, one that is almost cruel in its design. The relief you get from moving is real — and it is temporary. So the night becomes a loop: sit down, the urge builds, get up and walk, feel better, sit down again. People can circle that loop for hours. Then the alarm goes off and they are asked to be a functioning person.Y hay una segunda trampa encima, casi cruel en su diseño.
There is also a night-time partner condition worth naming, because it explains the bed-mate complaints: periodic limb movements of sleep — brief, repeated leg jerks during sleep that the sleeper usually doesn't know about. Cleveland Clinic notes that about 8 to 9 in 10 people with restless legs also have them. (Cleveland Clinic)
The iron story — and the number nobody tells youLa historia del hierro — y el número que nadie le dice
Now we get to the part that changed how I practice.
For a long time, restless legs was treated mainly as a "brain chemistry" problem. It still partly is. But the piece that has moved to the front of the line is iron — specifically, iron in the brain. The American Academy of Sleep Medicine, in its 2025 clinical practice guideline, describes low brain iron as an important underlying cause of restless legs syndrome, and the guideline deliberately elevates the importance of iron evaluation in everyone with RLS.Durante mucho tiempo, las piernas inquietas se trataron sobre todo como un problema de "química cerebral." (AASM)
Here is the twist, and it is the reason so many people are told "your iron is fine" and go home still walking their kitchen at midnight: the iron level in your blood and the iron level in your brain are not the same thing. Cleveland Clinic puts it plainly — researchers have found low brain iron in people with restless legs even when their blood levels looked normal. (Cleveland Clinic)
🤔 A perspective from my side of the desk — food for thought, not a prescription🤔 Una perspectiva desde mi lado del escritorio — para reflexionar, no una receta
"Normal" is one of the most misleading words in all of medicine, and restless legs is the clearest example I know. The blood test in question is ferritin — roughly, your body's iron savings account. For ordinary anemia, most labs flag a result as low somewhere around 30. So a person with a ferritin of 45 gets told, truthfully, that they are not anemic. Case closed.
But specialists treating restless legs work from a much higher bar. A widely used management algorithm published in Mayo Clinic Proceedings describes starting iron treatment when ferritin sits at or below about 75, or when transferrin saturation is under 20% — far above the anemia cutoff. Same number, two completely different verdicts, depending on which question was being asked.
So here is my honest framing, offered as perspective rather than instruction: "your iron is normal" is an answer to a question you may not have asked. If restless legs is disrupting your life, it is reasonable to ask your doctor for the actual ferritin number — not the word "normal" — and to ask whether it is high enough for this condition. One fair caution in the other direction: ferritin also rises with inflammation, infection, and other illness, so it can read misleadingly high.
⚠️ Please do not start iron on your own⚠️ Por favor no empiece a tomar hierro por su cuenta
Iron is not a harmless "might as well" supplement. Taking it when you do not need it can cause real harm, some conditions cause iron to build up dangerously, and iron pills are a leading cause of poisoning in young children — keep them locked away. Test first, treat second, and let your doctor decide which — and whether pills or an intravenous form is right for you.
If you take away one piece of news from this article, make it this one — because it is recent, it is significant, and most people affected by it have not heard about it yet.
For years, the standard first move for restless legs was a class of medicines called dopamine agonists (pramipexole and ropinirole are the common names). They work. Often they work beautifully, and quickly, which is part of the problem.
In its 2025 guideline, the American Academy of Sleep Medicine issued conditional recommendations against the standard use of pramipexole and ropinirole — a direct reversal of the 2012 guidance that had supported them. The reason is a phenomenon called augmentation: with long-term use, the medicine can gradually make the underlying condition worse. Symptoms start earlier in the day, feel more intense, and can spread — to the arms, to both sides, to hours that used to be peaceful. The AASM's own framing is that long-term use of these dopamine agonists is often associated with the risk of augmentation. (AASM) · (guideline)
What moved into their place? Two things, both worth knowing the names of so you can ask about them: iron treatment (including an intravenous form for adults), and a group of medicines called alpha-2-delta ligands — gabapentin enacarbil, gabapentin, and pregabalin — which the guideline notes are not associated with the augmentation seen with dopaminergic drugs. The guideline also puts real emphasis on a first step that costs nothing: looking for the things that stoke the fire, including alcohol, caffeine, certain antidepressants, antihistamines, and untreated sleep apnea.¿Qué ocupó su lugar? (AASM)
🛑 If you are currently taking one of these medicines, read this twice🛑 Si usted está tomando uno de estos medicamentos, lea esto dos veces
Do not stop it, lower it, or change it on your own because of an article — including this one. Stopping suddenly can cause a severe rebound of symptoms, and these changes need to be planned and tapered by the doctor who prescribed it."Conditional recommendation against standard use" also does not mean "this drug is wrong for everyone." The guideline itself notes that prescribing may still be appropriate for individual patients. Your doctor knows your history; a guideline does not.
What to do instead: bring it up at your next visit. A good sentence to use: "I read that the 2025 sleep-medicine guideline changed its position on this class of medicine, and that iron levels matter more than we used to think. Is my dose still the right plan for me, and what is my actual ferritin number?"No lo suspenda, no baje la dosis ni lo cambie por su cuenta por un artículo — incluido este.
What Delia actually didLo que Delia realmente hizo
Nothing dramatic. That is rather the point.Nada dramático. Ese es justamente el punto.
2. She asked for the number, not the word. Not "is my iron okay?" but "what is my ferritin, and is it high enough for restless legs?" Different question, different conversation.
3. She brought in the whole medicine shelf. Prescriptions, over-the-counter bottles, the lot. This matters more than people expect, because some very ordinary products can stoke restless legs — including antihistamines, which hide inside many "PM" or "nighttime" pain and sleep products. She had been taking one for the sleep problem the restless legs was causing. Neither of us stopped anything that day; we simply put it on the list to discuss, which is the right speed.
4. She stopped fighting it in bed. If the urge wins for twenty minutes, lying there rigid and furious is not a treatment. Getting up, moving, then returning is kinder and usually works better.
5. She protected the front end of the night. Caffeine moved earlier in the day. The evening glass of wine — which she had been using to sleep — turned out to be one of the known aggravators, which surprised her.1.
I want to be honest about what happened next, because I am not going to sell you a miracle. Her nights got meaningfully better, not perfect. What changed most was not her legs. It was that she stopped believing her own body was a character flaw. She went to a movie last winter. She stayed for the whole thing, and she made a point of telling me so.Quiero ser honesto sobre lo que pasó después, porque no le voy a vender un milagro.

Restless legs itself is not a medical emergency. But other things happen in legs, and a few of them are urgent. Please do not file these under "my legs are just restless."El síndrome de piernas inquietas en sí no es una emergencia médica. Chest pain or pressure, or sudden shortness of breath.Swelling or pain in ONE leg that is new — especially if that leg is warm, red, or tender. That can be a blood clot in a deep vein, and it is more urgent still if it comes with chest pain, breathlessness, or coughing up blood.
Sudden weakness or numbness in a leg, face droop, or trouble speaking — that is stroke territory, and minutes matter.
A leg that becomes suddenly cold, pale, and painful — that is an artery emergency, not a restless one.
Make a regular appointment — not "someday" — if your legs keep you from sleeping most nights, if the symptoms are creeping earlier into the day or spreading to your arms, if you are already on a restless-legs medicine and it seems to be working less well than it used to, if you are pregnant (restless legs is common in pregnancy and the safe options are different), or if you have kidney disease, which is closely tied to this condition. Also worth naming: leg pain that comes on when you walk and stops when you rest is a different problem entirely — that pattern points toward the arteries, and it deserves its own visit.
The one thing to carry out of hereLo único que debe llevarse de aquí
If your legs only bother you when you finally hold still — if the feeling is an urge rather than a pain, and walking is the only thing that answers it — that is not restlessness, and it is not a character trait. It has a name, and it is one of the most treatable things I see.
