A reference range is not a health target handed down by medical authority. It is not a goal anyone set for you. So: gather a large group of healthy people. Run the test on all of them. Line the results up from lowest to highest. Each test carries its own small chance of a false alarm, and the chances multiply across a panel. MedlinePlus says it plainly: "A test result that is within a reference range isn't always a guarantee of good health." Some reference ranges are wide — deliberately so, because healthy people genuinely vary. Take creatinine, one of the ordinary kidney numbers on a basic panel. At many labs the adult range runs somewhere around 0.6 to 1.2 mg/dL. A person whose creatinine reads 0.7 one year and 1.2 a few years later has very nearly doubled — and was never flagged, not once, because both numbers sat comfortably inside the box.Tome la creatinina, uno de los números renales comunes de un panel básico. That is why your doctor is reading the trend, not the dot. One result is a snapshot. Three results over three years is a story. Not every number on a lab report is the same kind of number, even though they are printed in the same font on the same page.No todos los números de un análisis son el mismo tipo de número, aunque estén impresos con la misma letra en la misma página.

A reference range answers: "Where do most healthy people land?" It is a percentile. It describes a crowd. Most of the results on a routine panel are this kind.

A clinical decision limit answers a completely different question: "Above what level do people actually start having worse outcomes — and where does treating help?" It comes from studying what happens to human beings over years, not from the middle of a bell curve.

Blood pressure thresholds, the A1c cutoff used to diagnose diabetes, cholesterol goals — those are decision limits. They were argued over, tested against outcomes, and revised as evidence changed. They are, frankly, the numbers that deserve most of your attention, and they are often the ones patients skim past on the way to the starred result that means far less.

So the only range that means anything for your result is the one printed on your own report, next to your own number. Here is a live argument inside laboratory medicine that I think patients deserve to know about, precisely because it is about a convention rather than about biology.

In Clinical Chemistry in 2024, Dr. Joe El-Khoury and colleagues questioned whether the central-95% rule should still be the automatic default for every test. They describe it as a "cookie-cutter approach" applied "with no specific consideration that each test may require based on the clinical utility" — and they point out something I find genuinely striking: the statistical convention traces back to nineteenth-century astronomy, to methods for locating a star. Their proposal is to tailor the rule per test — a 99th percentile for some, or clinical decision limits instead — so that fewer healthy people are flagged for numbers where, as they put it, there is no action to be taken anyway.

My own view, offered as perspective and not as instruction: I find that argument persuasive, and I suspect the reports we hand patients in ten years will look different from the ones we hand them today. What I am not saying is that you should discount a flag on your report because a convention is being debated. The current system is what your doctor is working with, and a flag is still how real disease gets caught. The fix available right now is not to ignore the star — it is to ask the better question about it.

Please do not read any of this as "flagged results don't matter." They matter. Flagged results are one of the main ways serious illness is found early, often before a single symptom appears.

Do not decide on your own that a starred number is "just the statistics." You cannot tell from the outside which flags are noise and which are the first sign of something real — that judgment needs your history, your medicines, your symptoms, and your previous results, which is exactly what your doctor has and an article does not.

What this article is saying: a flag is the beginning of a conversation, not the end of one — and panic is not a required part of it.

Nothing dramatic. That is rather the point.Nada dramático. Ese es justamente el punto.

1. He asked whether it was new. Not "is this bad?" but "have I ever had this result before?" It turned out he had — twice, years apart, at the same value. A flag that has been sitting still for six years is a very different object from one that appeared last month.

2. He asked for the number, not the word. "Normal" and "abnormal" are labels. The value itself, and where it sits inside the range, is the actual information.

3. He asked what would change. My favorite question in all of medicine: "Does this change anything we're doing, or are we just noting it?" Sometimes the honest answer is "we're just noting it," and that answer is worth hearing out loud.

4. He started keeping his reports. A folder. That is the whole system. It converts every future visit from a snapshot into a trend, and it costs nothing.

5. He asked about the numbers that weren't starred. His blood pressure — a decision limit, not a percentile — was the thing on that page that actually deserved attention. It had not been starred, because blood pressure does not come with a star.

And separately: if the lab or your clinic phones you about a result, take the call and follow their instructions. Laboratories are required to report certain critical values urgently. That phone call is not the same thing as a star on a printout, and it should never be left for later.

Make a regular appointment — not "someday" — if a flagged result is new for you, if a value has been moving in one direction across several years, if you feel unwell despite results you were told were normal, or if you are simply carrying a starred number around in your head and have never asked about it. That little range printed beside your result is a portrait of a crowd of strangers. It is useful — genuinely, enormously useful.
